Healthy BMI: What the Number Means and When to Go Beyond It
BMI is a screening tool, not a diagnosis. Learn what your BMI number actually means, its well-documented limitations, and the better metrics doctors use alongside it.
Your BMI is a single number derived from two measurements. It was never designed to be a health verdict. Adolphe Quetelet developed the formula in the 1830s to describe average population characteristics. not to assess individual health. Understanding what BMI actually measures. and what it cannot. makes it useful instead of misleading.
The Formula and What It Calculates
BMI uses a straightforward formula: weight in kilograms divided by height in metres squared.
BMI = weight (kg) ÷ height² (m²)
Example: 75 kg, 1.75 m tall → 75 ÷ (1.75 × 1.75) = 75 ÷ 3.0625 = 24.5
The CDC and WHO use four standard adult categories: underweight (below 18.5), normal weight (18.5–24.9), overweight (25–29.9), and obese (30 and above). These thresholds were set based on population studies linking BMI ranges to disease risk. They are statistical associations, not biological cutoffs.
What BMI Gets Right
At the population level, BMI correlates well with body fat percentage and disease risk. Studies consistently show that BMI in the obese range is associated with higher rates of type 2 diabetes, cardiovascular disease, sleep apnoea, and some cancers. At the underweight end, low BMI correlates with malnutrition, bone density loss, and immune suppression. These associations make BMI a useful screening tool for large groups and a reasonable first filter in clinical settings. It is cheap, fast, and requires no specialist equipment.
Known Limitations by Population
The same number means different things depending on who is carrying it. The CDC and WHO both document these limitations explicitly:
- Muscular individuals: Muscle is denser than fat. A competitive athlete with very low body fat can register as "overweight" by BMI. The scale measures mass, not composition.
- Older adults: Muscle loss with age (sarcopenia) means older people can have a normal BMI while carrying relatively high body fat and low muscle mass. a pattern associated with worse outcomes than BMI suggests.
- Asian populations: Research shows elevated metabolic risk at lower BMI values. The World Health Organization has proposed lower action thresholds (overweight at 23, obese at 27.5) for Asian populations. Some health systems now apply these adjusted cutoffs.
- Children and adolescents: BMI is not compared against adult thresholds for anyone under 20. Instead, it is plotted on age- and sex-specific CDC growth charts as a percentile. "Overweight" for a child means at or above the 85th percentile; "obese" means at or above the 95th.

Better Metrics to Use Alongside BMI
Clinical guidelines increasingly recommend pairing BMI with at least one additional measurement. The most useful options:
Waist circumference
Risk increases above 88 cm (35 in) for women and 102 cm (40 in) for men. Directly estimates visceral (abdominal) fat, which is more metabolically active and harmful than subcutaneous fat. Source: NIH NHLBI
Waist-to-height ratio (WHtR)
Waist circumference divided by height. Values above 0.5 indicate elevated risk across most ethnic groups. Research suggests it outperforms BMI as a standalone predictor of cardiovascular disease and all-cause mortality.
Body fat percentage
DEXA scan is the gold standard (±1–2% accuracy). Bioelectrical impedance (home scales) is less accurate but directionally useful for tracking trends. Healthy ranges: 18–24% for women, 10–17% for men (athlete level); 25–31% and 18–24% are "acceptable"; above 32%/25% indicates obesity by body composition.
What Your BMI Result Should Prompt
A BMI in the normal range (18.5–24.9) is a positive signal, not a health clearance. Someone can have a normal BMI and still have high blood pressure, poor metabolic markers, or a high visceral fat percentage. Conversely, a BMI slightly above 25 in someone who exercises regularly, has normal waist circumference, and good blood panel results may present little actual risk.
Use BMI as one data point among several:
- BMI underweight (below 18.5) or obese (above 30): discuss with a GP or registered dietitian
- BMI changed significantly without dietary or exercise changes: worth investigating
- BMI in normal range but waist circumference elevated: warrants metabolic screening
- BMI overweight but active and muscular: add body fat % measurement before drawing conclusions
